FSHP Inaugural Fellow of the Florida Society of Health-System Pharmacists

Purpose: Fellowship in the Florida Society of Health-System Pharmacists (FSHP) recognizes excellence in the practice of health-system pharmacy. Fellowship is awarded to pharmacists who have made significant and sustained contributions to FSHP and the profession of pharmacy. Fellowship status is for lifetime as long as the individual is an active member of FSHP.


Selection: Applicants must meet the criteria below. Applicants must earn a minimum of 60 points overall with at least 25 points coming from the “sustained contributions to FSHP”. A subcommittee of the Professional Affairs Council will review eligible applications and provide a recommendation to the Board of Directors who have the authority to grant or deny fellowship status. Appeals may be made directly to the President. Each year, the number of fellowships awarded will be determined by the Board of Directors.


Procedure: Candidates who meet the criteria below are encouraged to submit their application and their CV. Applicant CVs will be used to verify/confirm points for activities noted. Two letters of recommendation from pharmacists who can attest to the applicant’s qualifications should be submitted along with application, one of these letters must come from an FSHP member. There is no fee to submit your application for FSHP fellowship.


Award: Fellows will receive the designation Fellow of FSHP (FFSHP), a plaque, and will be honored at an award banquet.


Exclusions: Industry-sponsored presentations will not count for fellowship points. Poster and podium presentations will count only one time regardless of the number of times presented. Current members of the FSHP Board of Directors and/or Professional Affairs Council are not eligible to apply.
