Notice:  FSHP is required to inform you that 30% of your dues are applied toward FSHP lobbying activities.  Your dues may be tax deductible as ordinary and necessary business expenses subject to restrictions imposed as a result of FSHP lobbying activities.

Please select from one of the following Membership Classifications.  Members shall, at the time of application or at renewal, be classifed into the category of membership for which they qualify.

Please contact the FSHP office for details on reduced membership rates for these groups:

Residents:  Available to Active Members engaged in post-graduate residency training (general and specialized).

New Practitioner/New Graduate:  Upon graduation from a college/school of pharmacy, FSHP offers a six-month complimentary membership.

Husband/Wife Pharmacists:  When both husband and wife are eligible for Active Membership, FSHP offers a reduced rate for the couple.

Corporate Membership:  All pharmacists working for the same corporation may qualify for a bulk rate discount.
